Description:

Fuze HR Solutions is seeking an Agent(e) d’accueil et d’accompagnement to join a committed team working in the field of social intervention. The selected candidate will play a central role in welcoming participants, assessing their needs, and guiding them toward appropriate services and resources, ensuring they receive effective support during times of transition.

Key Responsibilities:

  1. Accueil et ouverture de dossier (20%)

  • Welcome participants, explain available services, and facilitate integration.

  • Complete required forms with accuracy and validate information as needed.

  • Open participant files following established procedures.

  1. Évaluation des besoins et plan d’accompagnement (40%)

  • Meet with participants to identify individual needs and objectives.

  • Develop personalized support plans in collaboration with each person.

  • Recommend orientations towards specialized services or external resources.

  1. Coordination avec les partenaires (25%)

  • Act as liaison between participants and internal/external partners.

  • Ensure regular follow-up with involved professionals.

  • Adjust support plans as participant circumstances evolve.

  1. Suivi administratif (15%)

  • Maintain up-to-date records in digital case management systems.

  • Produce required reports and evaluations.

  • Ensure compliance with organizational standards and documentation procedures.

Requirements:

  • Minimum 2 years of experience working with vulnerable populations (e.g., homelessness, addiction, mental health).

  • Proven background in client support, social services, or customer relations.

  • Knowledge of community and social service networks is a strong asset.

  • Languages: Fluency in French (oral and written), oral bilingualism in French/English required; knowledge of a third language is an advantage.

  • Technical skills: Proficiency with Microsoft Office and ability to manage digital case management tools.

  • Strong interpersonal, communication, organizational, and problem-solving skills.

  • Ability to adapt quickly, work autonomously, and collaborate effectively in interdisciplinary settings.

Schedule Options:

  • Sunday to Thursday: 8:00 am – 4:00 pm

  • Tuesday to Saturday: 12:00 pm – 8:00 pm
