Description:

Perennial is a healthcare provider that integrates physician- and nurse-practitioner-led primary-care clinics directly within retirement residences, offering concierge medical and allied-health services to older adults where they live. The interdisciplinary team includes MDs, NPs, RNs, RMTs, OTs, and dietitians, all dedicated to keeping seniors healthy, mobile, and out of the hospital.

As a Physiotherapist at Perennial, you will deliver hands-on, evidence-based care at partner retirement residences, starting with one or two on-site clinic days per month in Hamilton and Oakville (4- to 8-hour blocks), with the possibility of expanding your schedule as the program grows. There is also potential to pick up additional shifts at other locations if desired.

Key Responsibilities:

  • Perform comprehensive musculoskeletal (MSK) and neurological assessments for seniors and residence staff.

  • Deliver manual therapy, therapeutic exercises, gait, and balance training.

  • Design and lead individual or small-group fall-prevention and mobility programs.

  • Educate residents and caregivers on safe movement and home-exercise strategies.

  • Document detailed SOAP notes and outcome measures.

  • Submit electronic insurance claims and collect any resident co-pays.

  • Communicate progress and red flags to Perennial physicians, nurse practitioners, and allied-health team members.

  • Maintain a portable treatment kit and adhere to Infection Prevention and Control (IPAC) standards.

Qualifications:

  • Registered Physiotherapist in good standing with the College of Physiotherapists of Ontario.

  • Minimum of 2 years’ experience in orthopaedics, geriatrics, or community rehabilitation.

  • Strong manual therapy, exercise prescription, and patient education skills.

  • Valid Ontario driver’s license and reliable transportation; able to travel between sites.

  • Assets: Certification in fall management, vestibular rehabilitation, acupuncture/dry needling, or group-class leadership.
