Description:
SE Health is seeking a passionate and dynamic Speech-Language Pathologist (SLP) to join its Acute Care Transitions Program in London, Ontario. This innovative program ensures compassionate, coordinated care for patients transitioning from hospital to home. As part of a client-centered and interdisciplinary team, you will play a vital role in developing and implementing personalized care plans that address each patient’s physical, cognitive, social, emotional, and spiritual needs.
This position offers independent contract or employment options and follows a hybrid model, allowing flexibility to provide care both virtually and in-person in community or rehabilitation settings.
Key Responsibilities:
Conduct comprehensive speech and language assessments.
Formulate individualized treatment goals and make evidence-based recommendations.
Plan and implement goal-directed, client-centered therapy programs.
Collaborate closely with rehab assistants and interdisciplinary team members to ensure holistic care delivery.
Participate in joint care planning to support patient recovery and quality of life.
Maintain accurate documentation and adhere to professional practice standards.
Requirements:
Graduate of an accredited program in Speech-Language Pathology.
Current registration and good standing with the College of Audiologists and Speech-Language Pathologists of Ontario (CASLPO).
Experience in acute care, rehabilitation, or community-based therapy preferred.
Strong interpersonal, organizational, and communication skills.
Ability to work independently and collaboratively within a self-directed team.
Reliable, flexible, and creative approach to client care.
Knowledge of community healthcare resources (asset).
Proficiency in computer applications and clinical documentation tools.
Must possess a valid driver’s license and access to a reliable vehicle for client visits.
