Social Worker

Description:

Peel Region is seeking a compassionate and dedicated Social Worker to join their Communicable Diseases team within Health Services. This role focuses on supporting clients diagnosed with Tuberculosis and other health challenges such as HIV/AIDS and chronic illness. The Social Worker will address social challenges that impact treatment success, including housing and food insecurity, mental health, addictions, settlement issues, and access to resources.

The position requires delivering and coordinating social work services for individuals, families, groups, and communities. Responsibilities include conducting home visits, community outreach, counselling, assessments, crisis intervention, case management, collaborating with health professionals, mentoring staff and students, participating in program development, and providing court testimony when required. The role emphasizes trauma-informed practice, culturally appropriate counselling, and advocacy for marginalized populations including refugees, immigrants, low-income individuals, and 2SLGBTQI+ communities.

This hybrid role expects a minimum of two days on-site per week, with the remainder flexible within Ontario. The Social Worker will have the opportunity to contribute meaningfully to community health and public well-being while working in a supportive and inclusive environment.

Requirements:

• Bachelor’s or Master’s degree in Social Work and membership in good standing with the Ontario College of Social Workers and Social Service Workers

• Experience with high-risk clients and communicable diseases

• Strong assessment, counselling, and community engagement skills

• Valid driver’s license and access to a vehicle
