Resident Care Co-ordinator

Main Function: Direct, supervise and co-ordinate services for Resident Care within a Program Area.

Characteristic Duties: Under the general supervision of the Manager of Resident Care.
 

  • Responsible to recruit, supervise, complete job performance reviews, attendance management, train, educate and discipline within the clinical home areas on all three shifts (e.g. Registered Nurses, Registered Practical Nurses, and Personal Support Workers) in accordance with CGS policy and practice. Act as Management’s representative in the Grievance procedure in accordance with the respective Collective Bargaining Agreement.
  • Monitor and report on risk management/quality assurance activities, and key indicators to the Manager of Resident Care.
  • Conduct investigations relating to resident and staff incidents, medication errors, Health and Safety complaints and make recommendations to prevent future incidents.
  • Respond to Family/Resident concerns, conduct investigation and make recommendations towards resolving problems.
  • Receive, review and make recommendation for admission to the Program Area based upon functional assessments from the Northeast LHIN.
  • Responsible for Resident Care Programs as set out by the LTCH Act 2007 (e.g. Infection Control, Pandemic Planning, Wound Care, Continence Care, Minimal Lift Program, Pain Management, Palliative Care, Falls Prevention or Staff Development and other programs).
  • Collaborate with the multi-disciplinary team to ensure an inter-disciplinary approach to people-centred care.
  • Collaborate with the Disability Management Officer to recommend opportunities for injured workers to be involved in an effective Rehabilitation Program.
  • Facilitate professional services for Residents (such as clinical social work, occupational therapy, physiotherapy, dietician, medical physician, pharmacy, podiatry, audiology, optometry and other service providers).
  • Responsible for monitoring the Infection Control Program.
  • Responsible for reporting, investigating and completion of MOHLTC reports relating to Critical Incidents, complaints and annual inspections
  • Participate in the Accreditation Survey through planning, development, implementation and evaluation of processes.
  • Work evenings and weekends as required, and be included on the on-call rotation list.
  • Assume on-call responsibility as a Registered Nurse as required.
  • Responsible for materials management, ensuring equipment functions safely and identify equipment needs for budgetary expenditures.
  • Develop, implement and maintain policies and procedures specific to Pioneer Manor Resident Care, MOHLTC and Accreditation Canada standards.
  • Chair and participate on committees as required.
  • Responsible for completing funding requests (e.g. high intensity needs).
  • Develop and maintain a thorough working knowledge of CGS's Safety Manual and the applicable Provincial Legislation listed therein.
  • Perform other related duties as required.
     

Qualifications

Education and Training:
 

  • Registered Nurse in possession of a current Certificate of Competence as issued by the College of Nurses of Ontario (CNO).
  • Bachelor of Science Degree in Nursing and/or specialty in Gerontology from a recognized University with Canadian accreditation would be preferred.
  • Gentle Persuasive Approaches (GPA) or training an asset.
  • PointClickCare will be considered an asset.
  • Completion of a CHICA Canada Infection Control course or other courses related to infection prevention and control will be considered an asset.
     

Experience
 

  • At least three (3) years related supervisory experience in the Acute Care or Long Term Care setting.
  • Experience with electronic Resident Care Information System.
  • Demonstrate experience in supervision of staff and in labour relations.
     

Knowledge Of
 

  • Demonstrate knowledge of Long Term Care Homes Act, 2007, and related legislation and regulations, and their application to the Home.
  • Best practices in accordance with CNO guidelines.
  • Horizontal linkages to other relevant governmental levels and services as well as the private sector.
