Description:

RainCity Housing and Support Society is seeking a Level 3 Nurse to join the Foxglove Enhanced Health Housing team. The Nurse provides client-centered, strengths-based care to individuals with mental health, addiction, and chronic medical conditions. Working within an interdisciplinary clinical framework, the Nurse delivers flexible treatment services while ensuring integration and continuity of care across the healthcare system. The role includes intake, assessment, treatment planning, crisis intervention, and individual counseling while collaborating with service providers, clients, and their families.

Responsibilities:

  • Assess, plan, implement, and evaluate nursing care for clients in the Foxglove Enhanced Health Housing program.

  • Provide treatment and support for individuals with mental illness, addictions, and chronic medical conditions.

  • Work collaboratively with the interdisciplinary team and community partners, including Fraser Health services.

  • Conduct mental and physical health assessments and develop individualized treatment plans.

  • Provide crisis intervention, consultation, and individual counseling as required.

  • Ensure care aligns with CRNBC/CRPNBC Standards of Practice and RainCity policies and protocols.

  • Support medication management and psychosocial rehabilitation for clients.

  • Participate in program initiatives, including Indigenous cultural programming and peer support.

Requirements:

  • Bachelor’s Degree in Nursing (RN or RPN).

  • Current practicing registration with the BC College of Nurses and Midwives (BCCNM).

  • Minimum 3 years’ experience providing treatment services to individuals with mental health and addiction concerns.

  • Knowledge of harm reduction principles, mental health diagnoses, and support resources.

  • Understanding of oppression, marginalization, and culturally safe practices.

  • Ability to work independently and collaboratively within a multidisciplinary team.

  • Physical ability to perform outreach nursing, including light lifting (up to 15 lbs) and mobility in community settings.

  • Availability to work evenings, weekends, and some on-call shifts.
