Reporting to the Director of Quality and Practice, the Consultant will play a pivotal role in leading and facilitating initiatives related to quality improvement, patient safety, accreditation, and critical incident management. This position is integral in ensuring compliance with national best practices and regulatory requirements, while embedding a culture of continuous improvement across the hospital system.

The Consultant will be responsible for implementing and sustaining accreditation standards, supporting the Quality Improvement Plan (QIP), analyzing patient safety data, and leading evidence-based improvement strategies. A core responsibility includes facilitating root cause analyses for critical incidents, coordinating accreditation readiness, and supporting system-wide quality improvement initiatives. The role also involves engaging staff, providing education, and promoting a Just Culture approach to patient safety and organizational learning.

Key Responsibilities:

  • Conduct surveillance and analysis of patient safety incidents, quality indicators, and accreditation metrics to identify risks and areas for improvement.

  • Lead and coordinate critical incident reviews, root cause analyses, and accreditation tracers to ensure system-level improvements.

  • Collaborate with clinical leaders and staff to develop policies, procedures, and improvement initiatives that align with accreditation standards and organizational goals.

  • Facilitate the planning, monitoring, and achievement of the Quality Improvement Plan (QIP) to enhance patient outcomes.

  • Support accreditation readiness through evidence submissions, mock surveys, and tracer activities.

  • Provide project management and data analysis support for quality, accreditation, and risk initiatives.

  • Ensure compliance with patient safety and quality standards through continuous monitoring and reporting.

  • Educate and engage frontline staff in quality and safety initiatives.

  • Participate in quality, patient safety, and accreditation committees, representing the organization’s priorities.

Required Qualifications:

  • Undergraduate degree in a related field (healthcare administration, business administration, health profession).

  • College certificate/degree in clinical practice, risk management, or healthcare administration.

  • At least 3 years of experience in clinical risk management, patient safety, quality improvement, or accreditation within healthcare.

  • Experience with critical incident management and accreditation processes in the past 3 years is preferred.

  • Membership with a Regulated College in Ontario is an asset.

  • Certified Risk Management (CRM) designation is considered an asset.

  • Strong knowledge of acute care environments, quality improvement methodologies, and patient safety practices.

  • Skilled in data management, analysis, interpretation, and reporting for quality initiatives.

  • Ability to manage broad hospital-wide improvement projects with strong organizational and project management skills.

  • Excellent interpersonal, communication, and collaboration skills.

  • Strong computer literacy and familiarity with quality assurance systems.

Additional Information:

  • Work Schedule: Primarily weekdays, Monday to Friday (day shifts, subject to operational needs).
