Description:
REACH Community Health Centre is seeking a Pharmacist to join their interdisciplinary primary care team, focused on patient-centered care. Reporting to the Pharmacy Manager, the pharmacist will provide medication distribution and drug information services.
Key responsibilities include consulting with physicians, nurses, and other healthcare providers to develop therapeutic plans based on patient charts and clinical assessments, ensuring documentation aligns with regulations and practice standards. The pharmacist will provide patient counseling on drug therapy, reviewing reference literature and educating patients and their families on medication use and procedures, assessing their understanding.
The role includes pharmaceutical care and medication dispensing for a diverse client base, often medically complex with chronic diseases, concurrent disorders, or communicable diseases such as diabetes, mental health issues, or substance misuse. Collaboration with healthcare professionals to review and implement care plans is essential.
Candidates must be registered pharmacists with the College of Pharmacists of British Columbia, hold injection and OAT certifications, and have WinRx experience (an asset). Clinical experience, strong critical thinking, communication, and relationship-building skills are important. The candidate should be self-directed and enjoy collaborative innovation to improve community health. Multilingual abilities are a plus.
