Occupational Therapist

Closing the Gap Healthcare (CTG) is looking for dynamic and compassionate Occupational Therapists to join their team in various locations across Ontario. This role allows you to work in the comfort of clients' homes, using your therapy skills to improve their well-being and independence. CTG values flexibility, creativity, and adaptability in its therapists, offering a hybrid model of care that includes both face-to-face and virtual support. The successful candidate will have the opportunity to develop personalized therapy care plans, collaborate with clients’ families and other healthcare professionals, and contribute to a work environment that emphasizes teamwork, mentorship, and ongoing learning.

Key Responsibilities:

• Assess and develop therapy care plans based on clients’ real-life environments

• Provide health promotion programs, prescribe equipment, and promote client safety

• Use critical thinking to provide complex preventative or corrective actions for clients with diverse needs

• Partner with family members, caregivers, and healthcare professionals to set and achieve treatment goals

• Utilize a hybrid model of care, offering both face-to-face and virtual support

• Work independently while benefiting from mentorship, training, and team support

Qualifications:

• Graduate of a recognized Post Secondary program in Occupational Therapy

• Active registration with the Ontario College of Occupational Therapists

• Must possess a valid driver’s license and access to a reliable vehicle
