Description:

REACH is a nonprofit organization dedicated to providing accessible healthcare services to marginalized populations, including those experiencing homelessness, justice involvement, mental health and addictions challenges, and barriers to care. Reporting to the Clinical Lead, the Nurse Practitioner will deliver client diagnosis, assessment, planning, and evaluation within the full scope of nursing practice according to College of Nurses of Ontario (CNO) standards.

The Nurse Practitioner will develop and lead addictions treatment plans, act as a key resource for the justice team, support clients with social service applications, and provide advanced care tailored to marginalized populations. Advocacy and collaboration with community partners and justice programs are integral to this role.

The position requires working within a health equity framework to reduce health inequities and improve individual and community health outcomes. The Nurse Practitioner also contributes to program development, quality improvement initiatives, and participates actively in organizational meetings and strategic goals.


Key Responsibilities:

  • Conduct client diagnosis, assessment, and care planning under CNO standards

  • Develop addictions treatment plans and collaborate closely with Clinical Lead

  • Support justice-involved clients in accessing healthcare and social services

  • Provide care to marginalized populations including homeless, under-housed, and those in poverty

  • Advocate for clients at individual and systemic levels

  • Collaborate respectfully with healthcare professionals and justice programs

  • Maintain documentation using Telus PS Suites

  • Participate in quality improvement and program evaluation

  • Comply with relevant legislation including OHSA, PHIPA, AODA, and Human Rights Code

  • Maintain professional competencies, liability insurance, and CNO requirements

  • Work flexible hours as requested


Qualifications:

  • Nursing degree from recognized university or equivalent combination of diploma and training

  • Current extended class registration with CNO in good standing

  • Current professional liability insurance

  • Minimum 3 years clinical experience, community setting preferred

  • Minimum 3 years caring for socially and medically complex individuals

  • Certification or willingness to work towards addictions medicine

  • Proficient with Electronic Medical Records

  • Strong knowledge of harm reduction, trauma-informed care, mental health, addictions

  • Familiarity with Ontario Works and Ontario Disability Support Program

  • Excellent communication and interpersonal skills

  • Ability to work independently and within interdisciplinary teams

  • Commitment to anti-oppression and client-centered care approach
