Life Care Planner – Contract Position

Tri-Star Health Management Group Inc. is seeking an experienced and detail-oriented Certified Life Care Planner to join our team.

Position Overview

The Life Care Planner will complete comprehensive file reviews and evaluations to develop Life Care Plans outlining an individual’s current and future care needs.

These reports are used to guide treatment planning and support medico-legal matters.

Key Responsibilities

• Conduct comprehensive evaluations for future care needs
• Review medical, rehabilitation, and collateral documentation
• Consult with treating providers and interdisciplinary teams
• Develop detailed Life Care Plans outlining future care needs and associated cost
• Prepare clear and well-organized medico-legal reports

Qualifications

• Certification in Life Care Planning (CCLCP)
• Regulated healthcare professional preferred (Occupational Therapist, Registered Nurse, Physiotherapist, etc.)
• Experience working with catastrophic injuries such as brain injury, spinal cord injury, amputations, mental health, chronic pain and/or complex medical/rehabilitation needs
• Strong report writing and analytical skills
• Knowledge of Ontario auto insurance (SABS) and medico-legal processes considered an asset
