Licensed Practical Nurse

Description:

Sunrise Senior Living is seeking a Wellness Nurse (LPN) to join their team in Vancouver, BC. In this role, you will play a vital part in ensuring the health, well-being, and clinical care of residents in an assisted living setting. The position involves providing clinical leadership, monitoring resident health, overseeing medication administration, coordinating health services, and supporting overall wellness programs.

Key Responsibilities:

• Monitor and assess residents’ health status, conduct monthly wellness visits, and follow up on any changes in care needs, falls, or incidents.

• Assist the Resident Care Director (RCD) with evaluations, move-in assessments, and ongoing care planning.

• Coordinate with physicians, families, and healthcare providers to ensure proper medical care is delivered.

• Maintain wellness files, medical supplies, and emergency kits in compliance with regulations.

• Oversee and audit medication administration processes, ensure compliance with the Six Rights of Medication Pass, and liaise with pharmacies.

• Promote infection control standards, participate in quality assurance initiatives, and support engagement programs for both residents and staff.

• Provide training, mentoring, and clinical support to care staff to maintain high standards of resident care.

Qualifications & Skills:

• Graduate of an approved college/school of nursing.

• Current provincial license as an LPN/LVN.

• Minimum 1 year of experience in home health, assisted living, hospital, or long-term care.

• Strong organizational, communication, and decision-making skills.

• Proficiency in Microsoft Office and ability to learn Sunrise systems.

• Ability to work evenings, weekends, and flexible hours.
