Description:
Working in a hybrid capacity, you will provide robust, comprehensive primary care to patients across a continuum of settings—street health, shelters, SROs, supported housing, and complex care housing. Care will range from episodic and opportunistic encounters to longitudinal continuity, delivered in the ethos of meeting patients where they are and providing care without barriers.
This role involves close collaboration with embedded nursing and social work teams, often through case conferences and shared care planning. The position is well suited to clinicians with an interest in complex care, mental health, addictions, and the social determinants of health.
As a General Practitioner with Vancouver Coastal Health, you will
- Serve as Most Responsible Provider (MRP) for clients in complex care housing models.
- Deliver comprehensive primary care, including preventive, chronic disease, and acute care management, adapted to the realities of highly complex patient populations.
- Provide episodic and bridging care following initial nurse assessments—virtually or in-person.
- Develop episodic care plans with nursing staff and determine bridging to appropriate services.
- Collaborate with other physicians, NPs, outreach teams, and acute care colleagues to ensure continuity.
- Clinical activities may include
- Preventive and chronic disease management (e.g., diabetes, COPD, hypertension)
- Management of skin and soft tissue infections (SSTIs)
- Bridging prescriptions for opioid agonist therapy
- Antipsychotic titration in cases of psychiatric decompensation
- Rare virtual certifications (Form 4 under the Mental Health Act)
- Care coordination and discharge planning, liaising with specialists as needed

Education, Licensing, & Experience
- Current College of Physicians of BC (CPSBC) - Full
- Canadian College of Family Practitioners (CCFP) - Mandatory
- Canadian Medical Protective Association (CMPA) - Mandatory
- Emergency Experience - Desired
- MD with CCFP (or equivalent) and active license to practice in BC (CPSBC).
- Experience in primary care, emergency medicine, psychiatry, addictions medicine, or related fields.
- Strong proficiency in team-based care and comfort working with nurses and social workers.
- Familiarity or willingness to learn the HCCFAA, MHA, and AGA legislative frameworks.
- Commitment to patient-centered, harm reduction-oriented care.
- Proficiency in EMRs (Profile, PARIS, Cerner); training and compensation provided.
- Strong advocacy skills in navigating acute care systems and discharge planning
