Responsibilities:

  • Perform comprehensive client assessments and develop, implement, and evaluate individualized care plans based on best practices.

  • Act as the primary care provider for assigned clients, leading their care and collaborating with the healthcare team.

  • Support clients through care transitions by connecting them with local community support services.

  • Use professional judgment to manage client care effectively under the pay-per-visit model.


Requirements:

  • Current registration with the College of Nurses of Ontario (CNO).

  • Canadian Nurses Association (CNA) specialty certification is an asset.

  • Passion for community care and evidence-informed practice.

  • Current CPR certification.

  • Valid driver’s license and vehicle (travel required to Woodstock and Ingersoll).

  • Up-to-date immunizations, clear criminal background, and vulnerable sector checks.

  • Willingness to obtain Saint Elizabeth certifications.

  • Physical ability to handle lifting (up to 55 lbs), walking, climbing stairs, standing, bending, reaching, and frequent movement.

  • Must work 8 am to 5 pm depending on caseload, with evening or overnight on-call shifts 6 times a month plus every other weekend.


Return of Service Eligibility (if applicable):

  • Not employed as a nurse in Ontario in the previous six months.

  • Certificate of Registration in good standing with the CNO.

  • Employment must begin no later than March 31, 2025.

  • Commitment to 24 months full-time employment.

  • Not receiving Ministry of Health Tuition Support Program funds simultaneously.
