Community Health Services Manager

 

Description:

Cota, an accredited not-for-profit organization, is seeking a dynamic individual for the position of Manager, Community Health Services. This role is pivotal in providing leadership, clinical, and operational oversight to ensure the development and delivery of high-quality, client- and family-centered services for individuals facing mental health and cognitive challenges.

The Manager, Community Health Services will oversee the 11 Macey Ave. program, which includes 56 permanent affordable housing units for individuals who have exited chronic homelessness. The position includes responsibility for the 24/7/365 onsite support services, including an opt-in meal program. The Manager will work closely with the Supervisor at this site, managing and supporting the team to ensure the tenants maintain housing stability, develop a sense of community, and pursue overall health and wellness.

Key Responsibilities:

  • Leadership & Direction: Provide clinical and operational guidance, ensuring services align with Recovery Oriented, Housing First, Harm Reduction, and Anti-Oppression principles.

  • Program Development: Collaborate with the Director of Community Health Services to lead program development and quality improvement initiatives.

  • Partnership Building: Strengthen partnerships with The Neighbourhood Group (housing operator) to provide client-centered, solution-focused supports.

  • System Coordination: Improve coordination of services aimed at supporting tenants, including housing stability, crisis intervention, and community integration.

  • Staff Supervision: Oversee recruitment, training, and supervision of staff, providing both administrative and clinical support to frontline service providers.

  • Stakeholder Engagement: Build relationships with external partners and stakeholders to ensure optimal support for tenants.

  • Outcome Measures: Contribute to the development of program outcome measures and evaluations, analyzing data and implementing quality improvement initiatives.

  • Financial Oversight: Provide oversight of program budgets and approve expenditures according to policies.

  • Team Management: Support staff wellness and performance, including scheduling, performance evaluations, and staff training.

  • Standards Development: Lead the creation of program-specific operating standards and provide ongoing education to the team.

  • Problem Solving: Analyze issues and identify the best solutions to ensure service delivery remains effective.

Education and Qualifications:

  • Degree/Diploma from a recognized University or College in a related Health Care or Social Service discipline.

  • Current registration with the respective professional college (e.g., OT, SW, RN) is an asset.

Experience:

  • Minimum 3 years of supervisory or program management experience, especially in a community-based setting.

  • Experience in hiring, training, and supervising staff.

  • Experience working in partnerships with other agencies, especially within the mental health and addiction sector.

  • Experience in a unionized environment is a plus.

Knowledge & Skills:

  • Strong knowledge of Intensive Case Management models.

  • Expertise in evidence-based practices in mental health and addiction services, particularly recovery and harm reduction.

  • Demonstrated leadership skills in building teams and providing coaching and support.

  • Familiarity with electronic health record systems and reporting tools like OCAN and OPOC is an asset.

  • Proficiency in Microsoft Office (Word, PowerPoint, Excel, Outlook, SharePoint).

  • Strong clinical experience in providing services to individuals with serious mental illness, addictions, and complex health conditions.

Other Requirements:

  • Flexibility in working hours, including occasional evening shifts and one to two weekend days per month.

  • Participation in the Manager-On-Call system on a rotating basis.

  • Strong writing skills for preparing reports and reviewing financial data.
