Description:
SE Health is hiring Registered Nurses for its clinic in Newmarket, ON, offering supportive teams, flexible schedules, paid training, and opportunities for career growth. This position may be eligible for the Community Commitment Program for Nurses (CCPN), which provides a $25,000 incentive for a 24-month commitment to the employer.
The role focuses on providing client-centered care through comprehensive assessments, individualized care planning, and evidence-informed practices. Nurses serve as primary care leads for assigned caseloads and collaborate with care teams and community support services to ensure continuity and quality of care.
Conduct comprehensive client assessments, develop care plans, implement, and evaluate care.
Act as the primary provider for a client caseload, accountable for care while collaborating with other care team members.
Support clients through care transitions and connect them with appropriate community support services.
Follow evidence-informed practices to ensure high-quality care.
Current registration with the College of Nurses of Ontario (CNO).
Canadian Nurses Association (CNA) specialty certification is an asset.
CPR certification required.
Valid driver’s license and vehicle (for travel to client locations).
Updated immunization records, clear criminal background check, and Vulnerable Sector Check required.
Willingness to obtain required SE Health certifications.
Availability to work days, evenings, nights, and every other weekend.
Physical ability to perform duties, including lifting up to 55 lbs, walking, climbing stairs, standing, bending, reaching, and frequent movement.
CCPN Eligibility:
Not employed as a nurse in Ontario within the last 6 months.
Must hold a valid CNO Certificate of Registration.
Commit to a 2-year full-time employment schedule between April 1, 2025, and March 31, 2026.
Not previously participated in CCPN or receiving funds from other Ministry of Health nursing programs simultaneously.
