Care Manager

Description:

Sunrise Senior Living is seeking a compassionate and committed Careaide (Care Manager/Designated Care Manager) to join the team at Sunrise of Victoria, an assisted living community dedicated to enriching the lives of seniors. The ideal candidate will be passionate about senior care and eager to support residents in living longer, healthier, and more joyful lives by providing top-quality physical and emotional care.

As a Careaide, you will be responsible for hands-on caregiving duties in accordance with residents’ Individualized Service Plans (ISPs), ensuring each resident receives personalized, dignified, and respectful assistance in a home-like environment. You will also play a crucial role in maintaining compliance with health and safety standards, participating in enrichment activities, and fostering meaningful connections with residents, their families, and your fellow team members.

Key Responsibilities:

Resident Care & Support:

• Deliver direct care, assistance with ADLs, and emotional support based on individualized care plans.

• Monitor and report changes in resident health, behavior, and needs.

• Support residents with continence management, hygiene, and grooming.

• Participate in activities that promote resident engagement, wellness, and life skills.

• Uphold dignity, independence, and resident rights at all times.

Team & Communication:

• Attend and contribute to daily shift meetings and crossover reports.

• Communicate clearly with residents, families, and coworkers.

• Collaborate with other care and medical staff to enhance resident outcomes.

Health & Safety:

• Adhere to all infection control policies and personal protective equipment (PPE) protocols.

• Report hazards, incidents, or changes in resident condition immediately.

• Maintain clean, orderly resident rooms and communal spaces.

Dining & Housekeeping:

• Serve meals and assist with room or dining room setup and cleanup.

• Observe and report changes in dining habits or diet-related concerns.

• Support residents with laundry and general housekeeping tasks.

Reminiscence Care (Memory Care Unit):

• Engage residents with dementia in sensory and life-enrichment activities.

• Help create positive daily experiences using the “Creating Pleasant Days” philosophy.

Qualifications & Core Competencies:

• Minimum Education: High School diploma or GED (required by regulations).

• Certifications: CPR and First Aid certifications (as per provincial requirements).

• Experience: Previous senior care or healthcare experience is a plus.

• Strong communication, decision-making, time management, and empathy skills.

• Must be at least 18 years of age.
