Care Manager

Description:

Stay Home Forever is looking for a dedicated Care Manager to oversee the delivery of nursing and personal support services (PSS). The Care Manager will supervise the nursing and PSW teams, develop care plans, ensure regulatory compliance, and support staff development. This role is critical to ensuring high-quality care for clients and requires leadership, care coordination, and operational management.

Key responsibilities include:

• Overseeing and supporting nursing and PSW staff to ensure client-centered care

• Developing individualized care plans and collaborating with healthcare professionals

• Conducting chart audits and ensuring compliance with regulatory standards

• Managing team development, including onboarding and training

• Overseeing operational aspects like staffing, scheduling, and resource allocation

• Building relationships with clients and families, addressing concerns, and providing education

• Participating in an on-call rotation to support staff outside regular hours

Qualifications:

• Registered Nurse (RN) or Registered Practical Nurse (RPN) with active registration with the College of Nurses of Ontario (CNO)

• Minimum 3-5 years of nursing experience, with at least 2 years in a leadership role (preferably in home care or healthcare management)

• Strong knowledge of nursing and PSW regulations, scope of practice, and best practices

• Proven leadership and team management skills

• Experience in care planning, case management, and compliance auditing

• Proficiency with electronic health records (EHR) and healthcare management systems
