Description:
William Osler Health System, a top healthcare provider serving Brampton, Etobicoke, and surrounding areas, is currently hiring a Behavioural Interventionist to join their compassionate and interdisciplinary team. This role focuses on supporting patients with responsive behaviours stemming from conditions such as Alzheimer's, dementia, mental health issues, substance use, and other neurological disorders.
The successful candidate will work collaboratively with hospital staff, patients, and caregivers to assess behaviours, identify triggers, and implement effective behavioural interventions. This position plays a vital role in improving care delivery, managing transitions, and ensuring patients and families are supported during hospitalization and beyond.
Key Responsibilities:
Conduct behavioural assessments aligned with Alzheimer Society Peel best practices.
Collaborate with staff and caregivers to develop and evaluate behavioural care plans.
Monitor and document behavioural outcomes and interventions.
Deliver patient-specific coaching and training to staff.
Support smooth patient transitions from hospital to home or other care settings.
Facilitate referrals to internal and community resources such as the BSO and Alzheimer Society Peel.
Observe and analyze responsive behaviours, identifying triggers and suggesting solutions.
Participate in care conferences, huddles, and interdisciplinary collaboration.
Maintain statistical data and administrative reports regularly.
Promote patient independence and safety through targeted interventions.
Qualifications:
Diploma in a health care related field (Bachelor’s preferred).
Certification in gerontology or working towards it (preferred).
GPA (Gently Persuasive Approach) certification required.
Experience with prevention and management of geriatric syndromes such as delirium/BPSD.
Strong documentation, communication, and interpersonal skills.
Proven ability to work both independently and as part of a multidisciplinary team.
Flexibility to work across multiple William Osler sites as needed.

Work Schedule:
Day shifts (8 hours), including weekends and statutory holidays. Schedule subject to change based on operational needs.
